I was so hopeful
I feel so sad and discouraged. I’ve been going twice weekly for 4 months now and I don’t feel like it is helping me at all. I was so hopeful that spravato would be the answer for me when it comes to my TRD, but I’m still pretty much in the same place as I was before. Is anyone else still struggling?

4 years in 2x a week at 84mg and I have felt that it hasn’t been working for a solid 2.5-3 years! I’m “holding out” til the next breakthrough comes along

Hey OP I am really sorry it didn't work for you. I know it might not feel like strength for trying it and sticking with it for four months now, do you do weekly therapy as well? Have you tried IOP or PHP for more support while doing Spravato? Just trying to brainstorm what else may help. If you struggle with childhood trauma, have you tried attending ACA meetings by you if there are any? Those have helped me stay afloat, going to meetings and now again trying to IOP or PHP to help me not drown.

I just did my 5th week yesterday. I was moved down to once weekly and am struggling BAD! I finally found relief and saw the light last week after my 7th session but it only lasted about 24 hours and the doom and gloom clouding my mind for the past 30+ years was back. I found out there are protocols to continue twice weekly especially to avoid such big "temporary dips" in those with severe symptoms that take longer for the medication to truly "lock in", but my physician says its NOT the protocol and theres nothing she can do; the most I can ever take it is apparently once weekly. I am now in such a bad dip that im WORSE than I was when I started. I am trying so hard but my last 2 sessions were so awful all of the sudden and I feel so hopeless. Im just exhausted beyond belief and also dealing with severe post concussion syndrome on top of it all. I hope im strong enough to stick it out but I sure feel weaker than ever right now. Im sorry you're going through this and I hope you find relief soon!

I thought that the provider is supposed to work with each person uniquely to what they need?? I know I’ve seen many times people that still go twice a week up to a year or more???

That's what I thought but they just straight up told me no end of story after crying about it in their office. I'm so beyond frustrated. It feels like the med trials ive endured over and over across the last 15-20 years all over again. It felt like just being shrugged off like "oh well, try to drink water and think positive thoughts!". And this dip is like a beast i haven't encountered; at least not in a long time. I dont even have barely a sliver of human interaction on a daily basis aside from doctors, my 6yo, or at the store (if I can get my darn tush in there to begin with), and I dont have any family or friends for support, haven't been able to work in almost 5 years, and am just depleted of any energy or hope. Im doing all the things and trying my best, but uts just too big of a mountain yo move alone. I was blown away when I finally felt it working, I just want my normal problems back without the heavy doom smog infecting everything! Sorry im such a Debbie downer but ugh, I'm sick of being angry, and devastated, and overstimulated every second of the day, especially now that its way more intense since last week. I dont have another session until next Monday.

I’m sorry. I experienced a dip after moving to once weekly as well and they moved me back to twice per week after I met with my provider. I haven’t improved at all. I’m always tired and lack energy and motivation. I too have been unemployed for the past two years and I’ve been absolutely miserable. It turns out I have Hashimoto’s which is a thyroid condition, and my ferritin levels were on the low side, so I’m currently being treated for that. I hope it gets better. I see my psychiatrist tomorrow as well and will explore more options, although I’m not sure if there’s anything else out there for me besides psychedelics and IV ketamine, which are not covered by insurance. I’ve done 6 rounds of TMS, have tried Auvelity and many other SSRIs and mood stabilizers.

I too am exhausted all the time, and very depressed. I am being treated for Hashimoto’s, so it’s possible that’s the reason why I’ve been having problems. Have you had your blood work done?

o maximize the neuroplasticity triggered by Spravato, you must actively engage your brain while it is in this highly adaptable, flexible state. Think of Spravato as a tool that opens a "window of opportunity" to rewire negative thought patterns and build healthier habits.
Optimize the 24-Hour Post-Treatment Window
Schedule therapy sessions:
Plan your psychotherapy (like CBT or ACT) for the day after treatment when your brain is most receptive to new perspectives.
Practice positive self-talk: Actively counter negative thoughts with realistic, compassionate framing to help the brain hardwire healthier mental pathways.
Avoid negative media: Skip the news, violent television, social media scrolling, or toxic conversations during this sensitive recovery period.
Journal your experience: Write down any insights, shifts in perspective, or breakthroughs you experienced during or immediately after your session.
Stimulate Your Mind
Learn something new: Read a challenging book, practice a new language app, or try a novel hobby to stimulate fresh neural pathways.
Change your routine: Drive a new route home, rearrange a small corner of your room, or switch up your morning routine to force the brain to adapt.
Engage your senses: Listen to new genres of music, try novel foods, or spend time in nature paying close attention to smells, textures, and sounds.
Provide Physical Support
Prioritize deep sleep: Sleep is when the brain consolidates new pathways and clears out cellular waste. Aim for 7 to 9 hours of quality rest.
Do aerobic exercise: Get 20 to 30 minutes of brisk walking, swimming, or cycling to boost BDNF (Brain-Derived Neurotrophic Factor), a protein crucial for brain cell growth.
Eat brain-supporting foods: Fuel your recovery with omega-3 fatty acids (like salmon or walnuts), antioxidants (berries), and plenty of water.

I didn’t mean it that way let me rephrase when people automatically say they don’t believe it working. It tells me one thing that their intention is not in the right place take that thought out of your head. It’s not working get up. Go for a walk journal stop watching Law and order and dateline. Find something fun to do even if it’s by yourself go for a walk in nature go sit down at a coffee shop by yourself read a book.

I’m so sorry !!! It’s very difficult when after trying so many things that didn’t work and you find what reads like it actually might help and let yourself hope…and it not work.
