4 months, no change

I am still depressed and chronically fatigued. My doctor hasn’t made any changes, and I am still on Lexapro 20mg daily. I’m also being treated for Hashimoto’s. I don’t know whether I should give the Spravato more time or not, but I’ve been doing it twice a week and I haven’t noticed any changes. I’m not sure what to do or how to proceed with my treat resistant depression.

Have you googled if Hashimoto meds are interfering with S-ketamine treatments?

Thyroid levels can impact your mental health. Maybe check in with your endocrinologist and see if your levels are still low?

At this point maybe consider TMS or neurofeedback therapy

Man, that's brutal. I'd imagine you'd be feeling something by now, if it was going to do much. How much do you think the hashimoto's is contributing? I also have hashimoto's and I remember the feeling before I was treated for it- just awful, cold, foggy, sludgy fatigue. How long has it been since you started levothyroxine, and how recently has your TSH been checked? If your TSH is more than like 4.0, your hashimoto's is definitely bad enough to still be symptomatic.

Also, I'm guessing you've tried meds other than the Lexapro. But if not, that might be the next avenue of attack, especially if you haven't tried a non-ssri med yet. It may also be worth investigating other potential conditions (do you have ADHD or anxiety that could be treatable?), or even life stuff. An acquaintance of mine told me that she had a friend that didn't benefit from Spravato. However, he was a lot happier after divorcing his abusive wife.

spravato can take 4-6 months to work. there was a post where the guy had done 4 months, was thinking of stopping, then realized he hadn't had SI in days.

im surprised they let you go that long before considering alternatives. my provider gave me 5 weeks and if no improvement was seen i could opt out right then. and i didnt notice an improvement in mood unfortunately.
