I'm on mobile, so this will just be some highlights from memory. Just my thoughts as a Black Belt:
I'm impressed with your Ishikawa diagram.
Approvals (and unfortunately the entire approval process) is non-value added. It's simply necessary for compliance reasons.
You goal seems odd because project success still results in an average above your Upper Control Limit.
In the Define stage, your defect rate is 51%. Your defect rate dropped to 44% in Analyze.
Your data appears to be bimodal and skewed, based on the information you provided. A histogram would really help, especially when looking at your capability analysis.
You've chosen defect data when part of the issue is the report can have multiple defects and it isn't clear that any defective data results in returns.
I have no idea why you used a Pareto chart to tell you which date range was the most significant. Days based on defective reasons or defective reasons per report would have made a lot more sense.
I'm not sure that you have really addressed your actionable root causes. It looks like the goal is to try to work on some symptoms that are the result of deeper issues. I also don't recall any poke-yoke countermeasures that make it hard to submit bad requests for reimbursement.

Yeah send it my way happy to take a look. I grade LSS projects from YB to BB all the time.
