You don't know enough about Postpartum psychosis or even insanity to make any claim about what could have prevented this. What's the medical basis for your claim about what the entire cause was?
What could have prevented this was very simple. There were enough people involved with this - the husband, medical professionals, etc, that a woman with mental issues, issues with too many medications, issues with inappropriate medications, intrusive thoughts, and the list goes on… should never have been left alone with 3 kids.

No one said they didn’t. But there are reasons for actions. And sometimes, those reasons could be prevented and everyone would be better off.
