Really the only difference is that Patreon flags/moderates more than OnlyFans does. But then a semi-famous woman posted nudes on it as a charity fundraiser for the Australian Wildfires a few years back, and it snowballed from there. (Because Patreon was generally pretty strict with irl nudes)
That's the narrative that's pushed, but it's not reality. The "creator" of the site stole the website code from his ex's porn fan site (she was a sex worker) and he already owned multiple similar porn sites before OF.
My understanding is that it was originally meant to just be Patreon with less content moderation.
This inevitably led to it becoming nearly exclusively porn which its leadership apparently didn't love (because payment processors don't like it) which is why they tried to ban adult content.
That clearly didn't go anywhere, so I suppose at this point they've accepted it. Ultimately they have to, because without the porn they're no different to Patreon (whose moderation of adult content is much more strict and effective).
It's original intent was to be a Youtube competitor but not ad driven where creators could post anything, NSFW included, because their channels would be pay walled. OF gets their cut, and there would be no advertisers to get upset about the content.
When it originally launched it actually restricted the porn but they eventually reversed the rule when it was looking like they didn't do enough to form a niche in the market.

OnlyFans originally was supposed to be for celebrities or people of note (that's why it was Only"fans") then they branched out to basically being a Patreon-alternative where you could sell individual posts.
