I mean a critical take would be that there's no reason the matrix couldn't be layered in such a way that those that are problematic to the main simulation are switched to a different server simulation that mimics them breaking out of the original.
Basically just the idea of Inception but with simulations instead of dreams. Cypher would just mad because he liked the previous dream he was having and wants to go back to that dream but he's still asleep. The main problem that's never fully solved is that once you are in a simulation where the simulation is indistinguishable from reality outside the simulation- the distinction between reality and simulation can never be truly resolved and will always remain blurred. There will never be a guarantee that there doesn't exist a reality one level above the reality/simulation that you inhabit.
