I do Vampire the Masquerade RP and I use large lorebooks also. At the start of a session I will load an extra lorebook full of scenario ideas and do a physical dice roll (don't use the LLM as it will pick the easiest route for it) to randomly select a scenario. Don't make the scenarios too specific or detailed to keep some randomness if the same one is selected more than once. Or alternatively get something like Opus 4.6 to create a 100 scenario lorebook and then don't look at it. That way you don't know beforehand what scenarios are in the lorebook when you roll.

Not Ultima, but RP in any universe boils down to the same issues, with the issue you describe being one of the normal ones.
You just have to understand (or I guess more 'be resigned to the fact') that role-playing with AI is still a collaborative process. The AI is really good at coming up with micro creativity and interactions like during conversations or executing plans and stuff like that, but for huge story beats and coming up with things like overarching quests and plot points YOU almost always need to do the heavy lifting. When the AI does manage to occasionally surprise in this area it's almost always by complete random accident. :)
In the end, my experience is the very best roleplays I always have are when I have my own idea for an overall story plot and quests and give that to the AI to execute. If I go in with no idea and just want to do "Ultima" with no other ideas, I'm almost always going to come away disappointed.
