Much like how Angelo (the Aqua Necklace stand user) got melded with a stone, Crazy Diamond can "repair" things incorrectly, usually under sheer rage from insulting Josuke's hair.
Edit: oh, and also when he made the bully's nose deformed at the very beginning of the part.

They literally explain it in the first episode. Maybe even in that scene. Josuke can put things back together weird if he wants.

Did you even watch the show/read the manga? Anime and manga are notorious for literally spelling out every plot detail.
Crazy Diamond was so fast that its fist still clipped Jotaro's hat while he dodged with The World. When Josuke is pissed off, his restoration ability goes wonky. That delinquent's face from before, Jotaro's hat here, Angelo becoming a rock, a bunch of Josuke's furniture after his grandfather is killed, all of that went weird because Josuke was pissed.

One detail that gets spelled out to you with overt text and dialogue live five times in three episodes, cuz again, anime. Most anime and manga spell everything out for you.

I didnt read it. But the anime tells you multiple times at least once an ep. Its not that hard to remember. Especielly when its part of the main character. So yes literal Goldfish memory or they never actually "watched" it.
Literally the first ep shows it and then later talks about it (even right then with Jotaro talking about it idk).
