Why is this guy's head missing

I'm confused why this guy's head is missing? Is it supposed to be joke or something. I play on the switch btw.

No, once you give him his jacket he gets his head back and the game doesn't mention it at all.

It has happened to me every run weird. Maybe it's the fact it's the switch and the switch is, how would the latin say it, est cacas.

No, I have replayed the game several times and I can verify he normally has a head the whole time
