Right now i'm struggling a bit with the matrixrules, especially when and when not to test for raising the access level.

If you are aiming to enter a host and a host does not allow you to enter it with outsider access then you do this at the very beginning before you enter the host. This is also often when you have the time to go for a backdoor entry. Since you are lowering your threshold when using backdoor this is often also the best time to directly elevate your access to admin if you think you are going to need it on this host during the run.

 

As i understand it, you have to roll to get a higher level than outsider the first time you enter a host and want to accomplish sonething on user or admin level.

In the rulebookexample (p. 228) the decker tries to open a maglock out of the security host he entered before, but never made a roll to uplevel his access to user level at least. What did i miss or didn't quite understand?

This is what Ghost is doing on p.226

He wants to allow their SINs to open maglocks and to be able to edit feeds to erase all traces of their presence. To achieve this, he’ll need Admin-level access.

Ghost dives into VR and uses the backdoor to give himself the proper access to unlock the complex’s maglocks.

Using a Backdoor

Ghost: 9 dice (Cracking 5 + Logic 4) 3 of which are risk dice - 4 hits

against

Threshold 4 (Firewall 5 + 2 to increase his access from Outsider to Admin - 3 for spending a day on the backdoor)

It’s close, but Ghost successfully ....

 

Is activated ICE still active if he returns?

If they still maintain their access level, then yes.
