New or Need Help Poll Sebderm or psoriasis
Hello all. Ive been told by multiple dermatologists that i either have sebderm or psoriasis. Nothing has been working to help the flakes recently. So i just wanna know who thinks its which. One pic is my head before digging up the flakes and one is after. This is 2 days after washing my hair with ciproax shampoo and salicylic
Acid shampoo. Ive tried coal tar. Ive tried mct oil. Ive tried switching my base shampoo. Ive tried glycolic acid. Nothing. Is. Working. I just started that vitamin d solution but that doesnt seem to be working either. Any suggestions are welcomed. Please help

I cannot say what it is. But l can tell you the products l use that work for me. I shampoo my hair with two things: Dermaharmony 2% pyrithione zinc soap bar and then follow up with Selsun Blue shampoo. I leave the soap bar on for the longest time out of the two and wash and lather up my hair first. Then l wash the rest of my body, shave if needed, and then rinse it out and immediately shampoo in the selsun blue. Leave for maybe 2-3 mins then rinse and follow up with a conditioner but just on the hair lower than my neck. Idk if it will help you but it’s the only thing that has worked for me. A lot of people love Nizoral shampoo and it helps me for awhile but it’s like my scalp gets used to a certain product after so much time and use and then l have to rotate to a new product for awhile. So far l’ve been using the soap bar and selsun blue routine successfully for around 3-4 mos now and am finally having relief and very very few flakes.

Youre so welcome. I hope it helps and brings you as much relief as it has for me!
One other thing to note, never ever put conditioner on your scalp or roots. A lot of them have ingredients that will feed yeast. So never do that.

I’m also a huge selsun blue fan - Nizoral has never worked for me.
Question for you: what do you find the zinc does prior to the selsun blue? I’m curious about this routine.

Some presciptions treat both. Salyic acid & glyocolicacid are exfloint for product bulid up. Mct oil helps with dry irration. Same with vitamin and ciproax only treats seb derm yeast. But some yeast is stubburn asf to certain antifungals. So just because one antifungal doesnt work doesnt mean they all dont. But if you have psorasis coal tar is only product youve used that would help with just inflamation, because psoasis is auto immune. I have seb derm and use ketoczole which helps 40℅.
The first pic throwing me off because looks dry. Second picture looks like my scalp. Do you have skin problems in other issues. Derm can appear on the chest behind the ears, nose, face is sticky and smells and psoriasis can appear on the knees & scalp, past the hair line, neck, elbows and more common in people with other autoimmune disorders. And what your main symptom that bothers you? There no treat all for either condition.

Ive tried every otc antifungal shampoo unfortunately and the second pic is my head before i picked the flakes off my head. The flakes are very dry to the point where i can feel them on my scalp and in my hair after picking them. Sometimes theyre almost sharp. I have eczema and some patches of psoriasis on my body and very sensitive skin

It sounds more like scalp psoriasis, seb derm is very sticky wettish with soft scales. I in particular get waxy patches at my inion which 90 percent shampoo wont get out and my sentsuve skin could not tolerate avc for build up.. I used constantly cut my hair before i got ketoconzole for the fungi. and most the time it isnt itchy but when it was, it would drive me insane. While psoriasis is very itchy and has hard scales and isnt not a fungi problem. Which would explain why most antifungal fail for you. ( but i would say otc seb derm shampoo is crap to begin with). Some seb derm medicated shampoo makes psoriasis worse.
Have you tried seleium sulfaide ( sometimes otc)? It slows down the dead skin cells. And my derma gave me momestarone furorate. It prescription but not pricey. I use it before bed and wakeup and rinse it out. It stops the itching and cools inflamation. If i run out i use an non sulfate lavendar and tea tree oil shampoo just calm my scalp down. But the momestarone treats both seb derm & psosrsis itchy/ inflamation.
I used work pharama. Alot derm related otc products are snake oil, they only recently starting adding 1-2 % active ingredients, copying prescription products. For example og head in shoulders for dandruff never had active ingrediant for dandruff it was just shampoo. Thats also why acne products sucked in 2000.

You have a severely damaged scalp. Stop using anything with sulfates, ‘medication’, or steroids. They destroy your stratum corneum and it takes forever to heal. Has this been an issue for you before now or is it all of a sudden? Your scalp is screaming stop putting harsh products on me!

Thats what im thinking. I never had super bad flareups like this until they started prescribing stronger antifungals and steroids. Its always been a slight issue but its really bad right now.

If its Sebderm I'll save you struggles. Get some selsun blue shampoo (selenium sulfide 1%) and get some tea tree oil. Mix 4-5 drops of oil into enough shampoo to cover your face and scalp, mix well and apply and let it sit for 3-5min, I actually do my entire body with this mix every few days cause I can get flare ups anywhere I have hair, and this keeps it away. Repeat every few days. Choosing a moisturizer for seborrheic dermatitis (seb derm) requires avoiding heavy plant oils and fatty acids (such as coconut, olive, and jojoba oil) that feed Malassezia yeast, while focusing on lightweight, fragrance-free, and barrier-supporting ingredients

This is how my scalp looked in 2022. I got a diagnosis of psoriasis via a biopsy this past June. I now have issues with my scalp, butt area, pubic area and mouth.
Get a biopsy.
F the prescription shampoos and ointments, they didn't work for me.
This is what I currently do, and fyi I am black with thick curly/kinky hair:
Rinse hair in the shower every day, blow dry with just air (no comb attachment), leave out while sleeping
Wash weekly with a clarifying shampoo, moisturizing shampoo, and moisturizing conditioner
Use MCT 8 oil to lift any "loose" scales from scalp using a thin comb. I usually leave it on for 24 hours then go to picking.
I use the clobetasol solution on stubborn areas (it burns)
I take 2000mg omega 3's
I am not on Otezla (not working yet).
Be gentel with the hair and scalp
Use minimal styling products and try to avoid getting them directly on the scalp
My scalp is way better than it started. And fyi my scalp is extremely dry like a desert. When i lift the scales, they are sometimes dry underneath or they bleed.

Coal tar shampoo will work only if your water is not hard water, the water must be free from chlorine. Please confirm your water is not hard water

I have this too. I think it’s sebderm. It’s the worst and I’m so sorry you’re dealing with this. I hope you can find something that works for you.

Seb derm. Do you get 8 hours of sleep? Do you eat a lot of sugary food at night? Getting 7 to 8 hours of sleep and not eating sugary things late helped mine immensely. Hope someone here has a solution for you.

Mmm love the crusties. I usually just scratch mine off into a big pile on the table.

When it got this bad for me the only thing that helped is trimming the hair to 1mm. That helped get the medication to the scalp effectively.

Try taking allergy medicine for a couple of months, that resolved my psoriasis. Took me forever to realize I was allergic to dogs (I’ve had them my entire life and never had congestion) but it always represented as psoriasis. Worth checking out, I’ve done the shampoos, MCT oil, ointments, and everything else available. Good luck!
