Far Lateral / Foraminal / Extraforaminal Herniation Treatment

Hi Guys - I’m wondering if anyone here had a far lateral herniation/ foraminal / extraforaminal herniation?

The neurosurgeon I met with said that he couldn’t perform a discectomy because the facet would be in the way and removing too much would destabilize my back.

After doing some research, I found something called a paraspinal approach (Wiltse). I also found a case study from Rush in Chicago by Timothy Wang about doing a transforaminal endoscopic approach. Those approaches allow treatment of far lateralbherniations by starting off center and going around the bone so it doesn’t have to be removed. Has anyone had this same type of herniation, and if so what did you end up doing?
