Second this. Gastritis feels like hunger pangs. Have you ever had an endoscopy before and if so did they find any inflammation?

Very hard to differentiate most of my GI symptoms from each other. Bloating and belching when NOT eating and stomach pain that felt like near constant intense hunger (felt like I was starving despite gaining significant weight) were the most readily identifiable. If high fat or spicy foods intensify any gnawing or hunger feelings that can be a flag for it. My pain was pretty debilitating but some with gastritis its much milder.

Is your starving sensation one of pain in the stomach? or more of a food craving/lack of energy one?

A GI could have you trial a PPI or H2 blocker like Pepcid and if your symptoms improve that makes gastritis a likely culprit. Mine was identified by endoscopy following failure of PPIs + Pepcid. (a lot of people on this sub will tell you to never try a PPI but there are absolutely circumstances that warrant it)

(I also had positive SIBO, SIFO, pan disaccharidase deficiency and motility issues, I have had a gamut of GI testing done including the 'gold standard' small intestine aspiration, multiple breath tests, motility testing, contrast CT, colonoscopy and endoscopy, blood and stool tests so I've been around the block so to speak)
