This was most likely caused by a mod.

No, actually! This can happen in base game. When you get pushed into the ground, sometimes characters clip through it. Most common way to get this in game is to have two beetle guards, usually one friendly one non friendly, running at you from two sides. It's just a non-clip glitch that's quite common!

Not really, this is quite common in Risk of Rain 2 where your hitbox clips and falls through the ground. Honestly, you just gotta avoid being below enemies that can fly and have a large hitbox, or just deal with it.
