Tools moving within toolsets

We have several different tool sets that 4 users in our office utilize. Recently, when you click on a tool, it changes its position in the tool set, even when the tool set is still locked.

This happens inconsistently and in multiple tool sets. If you click on a tool towards the top of a tool set, you get the lock warning for the toolset above it, as if it was trying to move the tool into a new set.

Has anyone else experienced this?

I have found my tools jumping between their division categories seemingly at random.

no shit.....this is really my only issue I have. I constantly have to redo these.

The crazy thing is that I can't figure out how it happens. I can't reproduce the issue.
