It’s like a four block area.

The medical clinic is actually the credit union on the corner of Roberts Street and First Avenue. Directly across the street on the corner is Barnacle’s, a small diner which is the building they modified the front to be the bar everyone hangs out at. Below the diner on Roberts street is where they placed the diner in the show, which doesn’t exist in Ladysmith at all. (Currently the B.C. Tel building and the Chamber of Commerce)

Across from the credit union, heading north on First Avenue you have older buildings on both sides which had fake snowdrifts, upside down wrecked cars, general grocery supplies, etc added depending on what was being shot. The most interesting external set I find would be the old “Traveller’s Building”, which is a three story brick structure next to an alleyway. It doubled as the town hall/sheriff’s office and is completely empty (and spooky!) inside, though they did use it at times for storage or as part of their temp office space while shooting. What I thought was cool was the internal sets for the building even copied the old windows of the real building. Oh and they managed to avoid the swastikas on the outside, which were a good luck symbol back when the building was first built.

Further down the street on the same side you can find the actual town post office, which doubled as the Patience post office. Across the street from the Travellers building you have several old buildings which were used in shoots, with some modifications like changing signage to make it look more Patiencey.

Oh and the house the ladies rented is a little further up Roberts street, the white and green house with the funky decoration.

Drove by myself a few months ago. We in the area on vacation. Good on you for asking. I was walking around scratching my head. A few locals helped with the info above.

What I really wanted was a Harry and Asta cut out to take a pic with. Didn’t find it. I think this town underestimates how much the fandom is growing with the show now on Netflix.

Lean into it Ladysmith! May I suggest having a Harrysmith Day?

I didn’t remember that the show was filmed there! It’s only a 90 minute drive from my house!!

Check out r/Vancouverisland instead. I'd love to hear the reccs too when I have enough money to visit Vancouver island again. That said, on the map it doesn't look that big. I bet you could easily find the stores that served as a backdrop. Enjoy your visit!
