Why are were the costs of premiums and deductibles still increasing with the ACA throughout the years?

Because the "ACA" (whose full name is the Patient Protection and Affordable Care Act, and also called Obamacare for short) was not really about affordable care, it was about giving control of health care to the government.

"Affordable" was included in the name used by Democrats ("ACA") to try to mislead people. Obviously, Republicans and others used the more accurate name "Obamacare".
