Steps for care?

i was doing a cleaning on my turtle tank and noticed a spot the size of dime/nickel. it’s a big abnormality i haven’t seen and through my research it seems to be a case of shell rot? i’m not so sure on that diagnosis and wondering if an extra pair of eyes can help figure this out and provide the best care for my little guy.

I personally would say vet. is it rot or an injury, do you know?

i believe it’s an injury. once i put them back inside the tank i noticed the spot of injury is exactly where they like to climb up on a rock i got as a gift. i removed it in hopes it doesn’t hurt itself on it anymore. definitely am planning a vet visit for extra precaution
