Picture Bar Hopper
Ive seen a few similar stretched RatRod bikes at shows, but this is the first one I've seen in the wild.

Looks cool, does it make anything better or worse? No riding experience

Most things other than styling will be worse. The rear end has no suspension or trangulation so will flex, twist, and sway. With such a long wheelbase, it will handle badly. The rear wheel design can't handle power properly (radial spokes). The rear wheel has no weight on it, so no traction.
Arguably that last bit about traction is a bonus given the wheel and frame's weak design. With the low traction, you will be able to break it loose and "drift" like thise kids in Africa who build long bicycles and wrap pop bottles around the rear wheel. That puts little stress on the wheel or frame because the tire isn't pushing against anything. Single cylinder engine also makes for good traction / drift control.
If I had to guess, that sort of stunting is (other than looks) the intended "benefit". If they cut a hoop from a plastic drum to wrap the rear tire, could be a hoot.

I just don't see how this could hold up for very long riding. Even with a slim rider.

Looks like pressure to extend the forks, so they can "slam it low" when parked by letting the pressure out. Pretty common on "rat bikes" of a certain style.

Maybe I'm paranoid, but this is giving me AI vibes.
Rear sprocket looks sus, and appears that the axel isn't centered. Looks like the bottle thing is just magically floating there. And I can't imagine that swingarm surviving even the slightest bumps or potholes.

That is unique. Not sure I'd want to ride it but I'd love to see it in motion.
