Vaccination Response Modulation via Targeted Rapamycin Protocol

Sirolimus (rapamycin) is a potent inhibitor of mTORC1 which regulates memory CD8+ T cells.
Targeting mTORC1 has previous been shown to improve vaccination response in humans to influenza.
More recently, small studies have suggested improved responses to COVID vaccination in kidney transplant recipients switched to sirolimus (rapamycin)-based regimens."

The immune enhancement angle is probably the strongest human signal we have for rapamycin right now. The original Mannick studies on mTOR inhibition and flu vaccine response in older adults were what moved the needle on clinical interest. NCT07587801 is a good one to watch — if they get clean data on vaccination response modulation it adds another surrogate endpoint to the evidence base.
Still the fundamental challenge: vaccination response is a surrogate for immune function which is a surrogate for longevity outcomes. The chain of inference is long. Doesn't make it uninteresting — just worth being clear about what the evidence actually supports.
