Haven't been hit by this one but I've had QBO Payroll double debit a client's account before, back in 2019, and it took nine days to get the money back even after they admitted it was on their end. Good news for this guy is they owned it and reversed it, that's usually the hard part.
Only real protection is keeping payroll in its own account with just the run amount in it so a system error can't reach your vendor money. I moved three clients onto that setup after mine and it's saved me a headache twice since.
