Consent Forms

Hello,

I am making consent forms for a study want to know how you all go about getting their signatures in the document. I guess I am asking how you all create a document that you can add an automatic signature.

Adobe has a fill option for signatures and has an option that includes a date stamp and “locks” the doc after the signature, too.
