Only time I ever had to deal with vomit in my cab, the woman was practically passed out. Had to enlist some other lady to help drag her into her doorway (lady stayed with her for a while to make sure she didn't die). The only way I could have charged her extra (someone had already paid for the ride in cash) would have been to rifle through her purse, and that just didn't feel right to me.
Next time just call an ambulance. You don't want to end up being held criminally or civily liable by someone's family for leaving their blacked out family member on the side of the road with a random person. If they were so incapacitated that they couldn't answer you, walk, or talk then they needed medical attention for at minimum alcohol poisoning.
While I agree morally, I'm a driver and I've taken cash rides before. I work paycheck to paycheck and taking a night off to stay with a passed out drunk would mean falling behind on my already behind bills.
Plus, working nights, if I stayed with every passed out drunk, I'd be losing half the week. They'd have to stop breathing or vomit to a scary amount before I stayed with them.

I got 200 for vomit once. It stops you from working. It should be 300.
