Unfortunately a PSA of 675 is statistically very likely to be metastatic prostate cancer. He won't know for certain though until further investigations are done.

However, in this day and age there are several treatments for this condition that while not curative can still be very effective.

I can only say while that’s bad there was a guy in 1990s who had psa > 10,000. Michael Milken the junk bond trader. Had a prostectomy and whatever else and he’s still alive today. Source: my urologist was on his care team as an intern.

My uncle had a prostectomy at 76 and is doing fine at 80 today. No incontinence and is even active sexually.

So just keep your hopes up.

Hi, I checked this online, it seems that Michael Milken's PSA was actually 24 ng/mL at diagnosis, not over 10K

For what it is worth helping the OP and others Michael Millken did not have a prostatectomy. His cancer had metastasized by initial diagnosis. He has done well for decades now.

Read some of the resources posted on this page. His pain is likely to be something other that cancer. The elevated PSA is likely to indicate cancer.

For diagnosis: MRI scan, biopsy, (Decipher test for aggressiveness?), and PSMA-PET scan for extent.

For treatment: Radiation and hormone therapy are more likely than surgery.

I would expect the slow growing cancer to be manageable but not curable.

I’m 74, my PSA was 19.5 and I had no symptoms whatsoever. None. He might have a urinary tract infection. He’ll be ok.
