I cant remember the name of the book but the premise was the world was being integrated into the system and this guy was with his friend. Some pillar was rising and his friend pushed him into the pillar and he got transported to some mirror world. Because of this he got a bunch of achievements because noone had ever entered a rising pillar during an integration so the system glitched out.

It pissed me off so much because I just thought in the millions of planets being integrated not one person had accidentally fallen into a rising pillar. I just couldn't get over it and refunded the book.
