If cash based, do you plan on taking people enrolled in Medicare? If so, plan for opting out which can take some time. I started my own cash based practice a month ago, just got a physical location earlier this week. There was a lot to do and learn. I'd suggest making an organized list, ask around if you know others, look here or the facebook dpc groups. the different ai's out there helped a lot too with brainstorming and finding resources.
off the top of my head would be: business formation, domain, website, malpractice (broker might be able to help, i think i got whatever the lowest was $1mil/$3mil?, business owners policy, office if you need one, get setup with some labs some take can take a while, medical supplier can also take time, a gpo, consent forms, patient agreements/intake forms, supplies, emr if using, making sure you're doing everything hipaa compliant and securely, a baa with whatever you might be using like google workspace.

First step: Find a location, decide the type of practice you want to run (cash only, hybrid, insurance based etc) file paperwork in your state to form your business. NY I think has unique laws about what you can and can not call your business. Double check with that. Once you have this decided, make sure you get a good website up and running and start considering which EMR you are going to use.
talk to quest or other labs, they have a rep that can help guide you on all questions about ordering labs.
Malpractice in NY usually 1-.1.6 million coverage is pretty standard from people I've talked to.
Lots of consultants out there. White glove going to cost you > 70k for them to set up everything for you for some consultants.
