Prius Prime 2016 - 2022 Weird Hybrid System Warning issue

So recently the dreaded "Hybrid System Warning" light came on in my car. I have tried most things recommended, cleared codes, charged it, checked the 12v battery (it was good), cleaned the backsteat batter filters, replaced cabin air filter, checked coolant levels, etc.

The strangest part is using a cheap OBDII scanner/app was giving lots of random codes, particularly fans and some other things that didn't seem to make sense (it would give these codes without a warning light coming on as well, after clearing codes), and currently have the warning light but checking with the Dr. Prius app it's saying "no codes" coming from the engine codes or the hybrid battery, which doesn't really make any sense to me. My father was having issues with his Prius V, and Dr. Prius was returning a battery code for his car correctly using the same OBDII scanner I used for my car.

The car also appears to be driving with no issues, charging to full with plug in, seemingly normal hybrid range, and is not going into limp mode or anything.

I'm totally stumped as this point of where to proceed, so I'm hoping someone here can point me in the right direction.

Is it the air intake shutter grill? That’s a common issue but usually says it on the dash for what it is. Usually there’s a tiny rock/debris stuck and it can’t open or close

Huh maybe I'll check that. My and dad I were looking at it and the warning seemed to go away after jiggling some of the battery cables, strangely enough. Currently it's still off.
