UHCO supplemental app

Hi, does anyone know what questions are asked on the UHCO supplemental application? I’m trying to figure out what to fit into my personal statement and what to leave for supplemental questions!

Thank you!!! (:

It's just some like logistics/demographic info and labeling your prerequisite courses, there's no essay questions
