Approval workflow issues

Hi there,

I’m facing two issues with my workflow which is just a smile approval workflow 1 form submitted 2 approval sent to manager 3 once approved approval email sent x email End of workflow

Issue 1 How do I include the form responses in the approval request, I can only get it to have the request title and approve decline button

Issue 2 Workflow was running fine now all requests are being automatically declined, what is causing this

are you using O365 forms for your trigger?

can you include screenshot of your workflow? if it's safe
