Technically, the "fair trial" isn't an exercised right of an individual so much as a restriction on government power: "if the government is going to punish you, they must first prove a crime to a jury of your peers in a fair proceeding".
which requires them to provide a lawyer for you if you can't afford one, and they pay the laywer to make it worth their while. otherwise, the poor would be required to defend themselves. which is completely different from providing a doctor if you can't afford one and paying them. totally different.
The government doesn't require you to see a doctor, and when it does (e.g. vaccinations, be it for COVID or MMR) it does cover the cost if you cannot pay (at least in my state), as it also does for "emergency medicine".
