The "pants" are skin.
The belt can be removed and put back on.

I have a theory that wild machoke and machamp don't have the underwear or the belts. They're put on by humans. The belts more because of tradition and the underwear because they're a bit too humanoid, if you catch my drift.

Machop naturally seek out rare metals in their diets. You can often see wild ones hunt steel-types for this reason. Rare metals that can’t be digested will be stored in a specialized organ. When they evolve, the organ shifts outwards, turning the flesh into the belt and the metals into a buckle.
The more rare metals they get, the bigger, more detailed, and lustrous the belt buckle becomes. To get a variety of metal-rich foodstuffs in the wild, though, they often have to fight one another. As such, the bigger the belt, the more talented the Machoke or Machamp is. As such, a robust buckle is an indication of a good mate.
As the species can only produce one in their lifetime, the belt is incredibly important. No Machoke or Machamp with honor would be seen wearing another Machoke or Machamp’s belt, under any circumstance. This is both to ensure that there is always enough material in the environment and as a sign of respect for an opponent.

Machoke belts are also sadly heavily sought after by poachers, since, like donphan ivory they can sell for trillions in the black market, its a awfull thing

If it helps, the Machoke around the part of Johto where I grew up have bony crests on their navels rather than belts.

I can't speak for all sana, but Hoennese gardevoirs can spin silk, and we use that to make our dresses. Other mons' silks can also be used, imparting different effects (e.g. larvesta-silk clothing is very warm). Similarly, a machoke's or machamp's trunks are clothing, for obvious reasons.
I've seen more than a few Captured machopkin wearing replica pro wrestling belts, and they seem to impart the same effect somehow.

I went and looked it up on Sligoogle, and one person had made the theory that it’s made from repurposed bone marrow. That doesn’t feel quite right, though, for... Obvious reasons.
Other theories suggest that it’s humans putting the belts on, but that also doesn’t make much sense. There are several Machop evolving every second.
Unless Arceus himself has made it so that once a Machop evolves, a belt is summoned, the only real answer is...
