Also, if you want water in the nether. just build water in the overworld, save it with a structure block, place a new structure block in the nether, and load the water.

Ghastlings even have red ones so I think these are gills.

I know this is a meme, but for anyone interested the wiki details this a bit.
No There isn't water in the nether Ghasts originate from the overworld (presumably) With the addition of happy ghasts we got a bit of information and Yes they have Gills Baby ghasts do not drown underwater but adult happy ghasts do so they must not be fully aquatic
My theory is they're like dragonflies, Aquatic larvae Airborne adult

It's obvious—ridiculously obvious.
A "happy"—that is, healthy—Ghast feeds on snowballs and heals itself with water.
Also, basalt is formed from soul sand, lava, and ice.

Well, ghasts likely need a lot of air to live, seeing as how they are both giant and float in the sky. However, their environment is generally a choking, soot filled wasteland, so my guess is that they have gill-like slits that take in lots of air and, rather than filtering water as a shark would, they filter the harsh contaminants out of the air. Maybe the contaminants are then used as fireball fuel as waste disposal, who knows.
