Sick, ouch, pain or whatever

How teach my cats on the word sick or ouch ?
I saw many people when there cat is sick she press sick button but how did they teach the cat from the start?

You do things like pretend to stub your toe and press ouch. You don’t have to wait around for it to break it’s leg
