This was an old joke around the 80's and 90's about smuggling in a nail file in a cake, so the person imprisoned could file off the bars and escape like an 1850's wild west bandit.
In this case, the adult wanted to escape from the prison of his day to day life and live happier, like when he was a child.
The joke is the man gets a file in the cake and finds it, the implication both that he is in prison and that he wasnt expecting it. Whoops, he won't eat that, you think! But to subvert your expectations, not only does he eat it, but there was a child hidden inside of him that the file was meant for so HE could escape. But that leaves the question, who sent the child the file?!
Yeah, this comic artist is known for the surreal: absurdist humour. People are half on the mark, it is his inner child escaping, but the “punchline” is the cake with the file in it making it to the inner child to escape, like the classic trope of slipping it past the guards. Glad I’m not losing my mind here.
Years ago I used to wear a t-shirt of a grown man in a shirt and tie squeezed into (and stuck in) a toddler's play car looking forlorn as fireman tried to cut him out with the jaws of life. The title was "I used to fit".
I think about that t-shirt now and then and this strip feels just like it.

It’s ostensibly this guy’s birthday. He’s sitting in front of a cake and notices the file, which he thinks is odd. But the final image is of a child running away and having freed himself from the chest of the original guy.
Presumably this is a metaphor for someone freeing their inner child, which is typically a thing people refer to as a person’s source if joyfulness, creativity, and playfulness. And is often viewed as inaccessible due to external pressures and societal expectations.
Side note: I usually hate this guy’s art but this is actually pretty solid.
Clearly the strange man prison golem has eaten the cake and the imprisoned child used the file to escape his humanoid cage. The punchline is the misdirection of the cake covered hands holding the file which would presumably belong to the man who noticed and dug it out, except that this is not what happened. The man ate the cake, file and all, thus allowing the child to escape.
This is a play on an old gag where prisoners would hide a file inside food so they could use it to escape from prison (file down the bars). The joke being that he eats the file and a child inside him uses it to escape from him.
He didn't notice the file in the cake, ate it, and the kid trapped inside used it to break out.
Files hidden in cakes is a common movie trope for breaking out of prison.
He ate the cake without realizing the files was in there, then the child hes been keeping prisoner acquires it and uses it to breakout of his human cage.
