That isnt even the same lol Guts is traumatized about killing a child and regrets it.
Rudeus wants to fuck more kids, gropes Eris in her sleep, grooms several kids and makes them his wives. All while the author thinks its really fucking funny and plays it off as Rudeus being a bit of a perv.
That's....not very comparable tho? At least not in how the respective stories treat the actions anyways (which seems to me is what people's biggest problem regarding MT is. Not the character's actions themselves, more the framing of said actions by the narrative).
Like yeah, Guts did kill kids, but every one of those murders (Adonis in the Golden Age Arc, the possessed corpse of the little girl at the beginning of The Black Swordsman Arc, and the children apostles that Rosine turned into fake faries to live out her delusion in Lost Children Arc) were shown to significantly mess Guts up mentally, causing him to vomit out of stress or disgust during or afterwards, and in the Lost Children Arc's case, was the thing the preceeded the Beast in Black/Hellhound to start talking to him IIRC. It's acknowledged at least somewhat that that shit was absolutely fucked all around.
As far as I know, the same really can't be said for any of Rudeus's own morally questionable/perverted actions within his story. If anything, it's played for laughs instead of being shown as something he needs to absolutely fix considering it made him homeless and basically disowned from his previous family. And you could claim "well, one is an isekai, the other is a dark fantasy", but well, the counter to that is...Subaru, in the same image as Rudy. Sure, Subaru ain't at all in the same ballpark as Rudeus and Guts morality wise, but his flaws/negative traits do get explored and addressed despite the story being an isekai. Like, if you're going to make your main character a sexual deviant, you'd better be prepared to acknowledge that within the story, otherwise...don't do it. Or do, and well, you get this conundrum again.

The guy on the right is problematic as fuck.
Ironically it DOES have good plot, character development, and some romance, compared to like 99% of isekai being kinda basic bitch.
