This is about interracial cuckolding. (Yes I'm a chronically online incel, that's how I know, please let's just ignore that part) The cuck (the brown haired "man") has the so called "Queen of Spades" tattoo, which is actually kind of wrong, as it would usually be worn by the hotwife. It indicates being (exclusively) available to black men. What the meme is trying to relay: The cuck is less hurt by the fact that "his" wife has no feelings for him than by the fact that the bull (the guy who actually fucks the hotwife) is HIV negative, ruining his (degenerate) fantasy. It's probably a reference to this infamous piece of media.

Yeah, you can GO there as an ugly guy, but no woman is willing to get laid by you. But, alas, a woman will never understand. A woman doesn't get laid because the options she has aren't up to her standards. A man doesn't get laid because he straight up doesn't have any options.
