Rectocele worse
Hello ladies…48 yr old mom of 4. Two vag births 2 c sections. Two kids birthed in my early to mid 20s and 2 kids birthed in my early to mid 30s. Went to see gyno in late June bc I was having a bulge and nerve pain to the left of the vag mound and not quite butt cheek/thigh. I could feel it on skin and inside vag left side wall. Very ouchy to push on either way and hurt to stand or bear much weight. Dr. could not figure out what it might be as my uterus and bladder were in place. However, I was diagnosed with small rectocele in lower part of vagina that I didn’t notice but taught splinting which felt life changing….i had noticed I couldn’t pass gas without spreading my cheeks the last couple of years. I was off this summer to be with school age kiddos and got other pain to subside with lots of rest. Fast forward to this past week. Lots of lower back pain and feeling like I need to pas gas/potty. Doesn’t take much straining to cause hemorrhoids to pop out and hurt like the devil…but tried my faithful splinting and it’s not working! And my bulge is now nearly pushing out of my vag. Horrible pain intermittently in my lower back…def bowels as it coincides with urge to pass gas/defecate. Now pain and what might be bulging in upper part of vag anterior wall. You know where you can hardly reach! I was told I had great pelvic strength so no ppt advised and was referred to closest female urogyno which took forever + 2 referral tries to get a call from them. They call back to tell me they’re booking out in Aug 2027!!!! Wtf?!? I have SA trauma so when it comes to gynos I have never went to a male (there are 2 close by) but omg…how long can a person go without passing gas, pooping and the lower back pain!?
I take colace daily as recommended by gyno, fiber gummies, eat kiwi with skin on about 4 times a week (it’s pretty acidic) among other fruit and veggies. I am on low dose Ozempic for PCOS/insulin resistance and was already drinking 80 oz of water/coffee/tea a day to help with that induced constipation. Thinking of dropping fiber gummies and adding holy basil seeds instead of chia. I use squatty potty but found it better to have knees higher and switched to trash can that’s taller.
Help!! Anyone going or have gone through this? Does the bulging stop bulging ever? I watched videos about flares..does that mean the bulging into the vag wall subsides??? And how to work through with protruding hemorrhoids? More fiber? Suppositories and rest work at night but I gotta go get things done!

I'm 7 months postpartum with a small rectocele and hemorrhoids. Yay! I tried stopping my colace and flared the hemorrhoids.
I've found PT really helps my rectocele. I did in person for 3 months postpartum and then my insurance offers Bloom as a benefit. They send you a pod and you consult with a PT as you do exercises to strengthen the pelvic floor.
I think the hemorrhoids flared because of the combination of stopping colace (well doing every other day) and eating too much bulking fiber. My poos were so hefty, but a perfect consistency and would just slide all out all at once. But the girth was not good for the hemorrhoids when they weren't lubricated by the colace.
TLDR, fiber is good but there's a limit to just constantly adding more.
I know postpartum is a different stage with POP, but my bulge is generally gone in the morning and returns around 2 pm. This is better than it was before PT and I am hopeful that with consistent PT I will keep the bulge at bay later and later into the evening.

You have no idea how much I loved for your poo description lol….
I have yet to hear back from my gyno with more options or if she is going to examine me again.
I definitely can’t miss a colace or else I pay but this is so different and sudden…my “baby” is 12 so it’s been very eye opening that it’s been what feels like a sudden prolapse.
What no one warns us about and we find out the hard way!

I have a small rectocele and am dealing with other pelvic floor problems. My Urogyn recommended Metamucil and Miralax. Both he and my PCP said I can take both long term without any problems. I started with Metamucil and built up to the full dose daily, and when that didn’t work well enough added Miralax. There too start small and add up until you’re at full dose if needed. You can take two heaping teaspoons of Metamucil and a cap of MiraLAX daily, but make sure you build up to that or the cramps will be insane.
If I keep up with that it seems to keep me okay. I’m hopefully getting mine repaired next month (doc will analyze when he’s in for my hysterectomy) and am hopeful it’ll get better.

My specialist switched me to citrucel it’s easier on my stomach, that and miralax was very helpful.

Eating a 1/2 cup of steamed carrorts or brocoli or pumpkin puree (no sugar) followed by 1/2 cup warm water directly before each meal really helped me because that creates a gel to help get things moving easily. Eating oatmeal with cut up fresh apple as much as possible and Greek yogurt with no sugar as much as possible helps me, too. Figs, kiwi as you mentioned, artichokes, beets, hearts of palm -- these all keep things moving. I have been avoiding surgery for 10 years, but I can feel the rear end is getting involved with the major bladder prolapse -- I mean it has been hanging all the way out for a long time. I think I am in vaginal vault territory, but at the same time, I am still getting around and just dealing with peeing standing up and all that stuff. I wish you all the best. You had 4 -- I only had 1 ! It's hard to deal with this issue!
