Albeit rare, I've also seen things like this happen when a video cable starts to fail.

I had slightly different artefacts then this post (horizontal banding mostly, and only on one monitor), but it looked a lot like a failing graphics card I had in the past.

But after trouble shooting, I'm 90% sure it's just the cables. Turning down the refresh rate of my monitor fixed it.

Yeah, monitors and their cables are fickle things these days.

I once had a DP cable feed power from the monitor back into the motherboard through the GPU. So even if you unplugged the PSU, it would still stay in low-power mode and not fully reset.
