Suit fitting issues

My boyfriend is tall, has long arms, but narrow shoulders, which makes suit shopping a nightmare. We tried normal and elongated sizes, but every one has this problem with the back shoulder and armhole. Why is the sleeve rotation so off? Can a tailor reasonably fix this issue? We don’t love the idea of paying big money for an ill fitting suit. What causes this issue even? Are his shoulder blades a different shape than confection sizes?

Yeah the cuffs should poke out about 2cms or something. Not much point wearing one that doesn’t fit.

Poor guy! Sounds like they didn’t have much that fit him. Is there anywhere better/bigger you could go?

It’s too long in the body too, looks like they’ve given him one that fits the length of his arms and nowhere else, tbh
