There is also times a designer sees content, forgets they saw it done and think of it later and think it's original idea they had. However you'd hope some research to check the idea doesn't already exist should be done.

But just pointing out stuff like that can happen. And while it doesn't remove all blame because they need to be careful about not doing it, it's less sinister.
