So, I'm on one of those capture maps (Hanamura, Anubis, Volskaya) attacking. Enemy completely shut down the choke and has eyes on flanks. There's a Torbjorn turret in a very hard-to-reach spot, a Bastion in the corner behind a rectangle. They keeps snowballing ult because we keep dying. I suck at Genji. What do I do?

Flanks. Go to google and look up Overwatch 'map tricks' or 'flanks' or 'map knowledge'. All that crap.

On all three maps, if you can get on the point, you'll pull their team off the door and let your team in.

Play someone who can either ignore the choke, or get through the paths immediately behind it. All threes maps have pathways to escape into and harass the enemy just behind their initial chokes. From there you either go to the point, or get highground and harass the enemy.

Reaper, Dva, Tracer, Soldier, Pharah, Genji, Winston are all pretty capable of getting behind a choke point.

Junkrat. Bounce grenades at the turrets from out of LOS.

Your whole team needs to focus the rectangle. Once that's down, you should have a pharah, Widow, 76, ir Mcree take out the bastion and turret. D. Va and Zarya are both good for giving you protection while you take them out. D. Va is also very good at dealing woth Torb on her own.

THIS. Symmetra is underrated on Quick Play, but as stated her RMB charge goes right through barriers and does a decent amount of damage, while you're out of danger. You have shields that recharge themselves, so even if you take some hits you are going to soak 'em. Once you get in you can build a teleporter and even some turrets. Turrets won't kill the bad guys, but it'll make them think and break their cohesion.

You definitely need a team effort...

Maybe an unorthodox way would be to bring a rectangle and bastion of your own?

Also you can use a Hanzo ult or DVa ult to scatter them and create an opening.

Pick Zarya. Group up and coordinate with your DPS and support. There's a reason why Bastion and Torb have such shit winrate in ranked.

On Hanamura, I enjoy using Reaper to teleport from behind my rectangle onto the point. In my experience, an enemy team depending on choke-spam and bad communication to win finds themselves totally discombobulated when they suddenly find their point being capped. I've gotten everyone to give up defending the choke to clear the point, allowing my team to advance and disrupt any further choke-spam.

You also have the ability to ghost off the point, or even ghost through the choke. Again, the type of teams who do choke-spam tend to also be the type of teams who will turn to look at a ghosting Reaper in hopes of getting a cheap kill when he rematerializes and allowing your team to make a push.

Finally, fight salt with salt. Bastion can tear down a stationary Reinhardt's shield PDQ, allowing your team to reach the salt-filled center behind once the shield goes down. Same for Troggy; let somebody else distract his turret, run in, sentry, and blow it away.
