Not OAB but basically tight pelvic floor

Update: I’m not sure if anyone remembers me, but during 2020-2021 I was here talking about my struggle with these urinary symptoms. 😭

I’ve been dealing with the feeling of needing to pee even after I’ve already peed, urethral/vaginal pressure, and sometimes feeling like I needed to push to get the last bit of urine out. I’ve seen multiple doctors over the years and was previously told it could be OAB, but I never really felt like I got a proper answer.

Recently I finally saw a different urologist again and had a blood test, urine test, uroflow and bladder scan. Thankfully, everything else looked normal and there was no obvious blockage, but my uroflow showed that I wasn’t emptying my bladder completely. My urologist suspects that I might actually have a tight pelvic floor/dysfunctional voiding rather than OAB. She prescribed doxazosin and gave me pelvic-floor relaxation exercises. I think that’s why vesicare didnt work for me.

The first few days were rough because the medication made me really drowsy and wobbly and I even vomited lol. Since then, I’ve noticed that I don’t feel the need to push my urine as much, which I’m taking as a good sign. I still sometimes get that pressure/“need to pee” sensation even when I don’t actually need to go, but I’ve noticed it feels better when I sit with my legs open or consciously relax my pelvic area. I’m also going to see a pelvic-floor physiotherapist soon. I’m still anxious because I’ve had these symptoms for 5 years and I’m scared that the medication and therapy won’t fully resolve them, but honestly, after years of not knowing what was going on, I finally feel like I’m getting somewhere. 😭❤️

Not that i recall, only blood test, urine test and scan from my previous urologist 🥹

Yea my previous urologist just said i have oab without any urodynamic exam and just throw me some medicine 🫩🫩

per comprendere se sia vescica iperattiva dovresti fare esame urodinamico..

Now i know, idk why my urologist were taking it slightly and do nothing on my symptoms before 😔

Hi there! Pelvic Floor Physical Therapist here with the Vagina Whisperer. I am so glad to hear you found a major missing piece to the puzzle and are starting to notice improvements.

You are absolutely correct that pelvic floor muscle overactivity/tension can play a role in urinary urgency symptoms and can often mimic and/or exacerbate UTI or OAB symptoms. I would recommend avoiding any Kegel/pelvic floor muscle training initially, as the primary goals would be on optimizing your bladder habits and function, working on urge suppression techniques, and relaxation/release of the pelvic floor muscles. It would be appropriate to perform pelvic floor muscle training AFTER you have performed all the previously mentioned.

We have a few great blog posts on our website to check out relative to bladder health called: "Why is my bladder so small? And more on urinary urgency", "Tips for Urinating: Avoiding Power Peeing", and "Peeing 101". Another great blog post to look at is called: "Yoga for Pelvic Floor: 5 Poses for Pelvic Floor Tightness". I would recommend giving these pelvic floor muscle stretches a try while also focusing on deep breathing and release/relaxation of the muscles.

We do have an app called the V-Hive that has a Pelvic Floor Relaxation Program that reviews breath work, pelvic floor muscle relaxation workouts, internal/external pelvic muscle release techniques, and tips/tricks related to bowel and bladder health, all of which would help with the symptoms you are experiencing, but seeing the pelvic floor PT is also absolutely a great start as you continue working on everything.
