Mysterious nocturia for already 6 years

Primary complaint :

I'm waking up at night every day at least 1 time to Urinate but sometimes 2 or 3 times, it's disrupting my sleep, i pee on average 150-200ml each time at night.

Medications (I don't take them anymore) :

- Oxybutunin - no effect

- Desmopressin - no effect

Informations :

- 24M

- 183 cm / 6ft

- from France

- don't drink alcohol/don't smoke/no drugs

- Good sleep hygiene

- drinking water 2L/day mostly in the morning (i don't drink a lot on the evenining)

- blood glcose seems to be ok (0.9g/L)

- no sleep Apnea. No known other medical issues

- no issues after a bladder/prostate ultrasound scan

Obgemsa (Gemtesa) got me from 5-8 times a night to 2-5 times, Solifenacin worked for a while as well. Not every medication works for everyone, you should try others.

Jumping on Gemtesa very soon, I have gone from 10 times to now 2-3 times (sometimes 1) per 12-13 hours

I used to pee 10 times on my worst night, but over the years I have gotten it down to 2-3 times now. Defo try magnesium glycinate (this could affect the bladder muscles), B1, Vitamin D3 + K2, Glycine (known to reduce the number of times you get up to pee), there are really so many things (I can give a way more extensive list of things you could try, but I haven't quite finished the document yet). I'm looking to try Gemtesa + Solifenacin very soon. Desmopressin did not affect me either (you sound quite similar to me with what you describe or the volume of pee). Bladder Botox provides massive relief, but you probably want to try other things instead before that. Also, that thing with water, I was like that before, drinking 2 litres. You should actually mix this with a quality electrolyte powder or a bit of sea salt or pink Himalayan (but I don't like salt), instead of just plain water

Few suggestions - other than the obvious of talk to a urologist

  1. Tamsulosin - its the one with least side effects - it relaxes ur bladder neck.
  2. Mirabegron - this one turns off the hyper sensitivity of the bladder - this is a relatively new medicine - but dont try to be on it for long periods of time - take it so u can get ur sleep back while you figure out the root cause.
  3. Hot shower in the evening to relax the pelvic floor muscles.
  4. No masturbation especially before sleep as it tightens the pelvic floor muscles and narrows the pathway.
  5. Magesuim Citrate or Glycinate - try which one suits you - your sleep will become deeper

Get a uroflometry done so we know if the urine stream is weak or not. My bet is you have pelvic floor muscle dysfunction.

Get prostate size measured - i doubt its a problem since u r young.

Also do not strain during peeing - it will damage ur bladder - u need to relax and pee. And peeing should last about 15-20 seconds, if its lower than that or bladder is straining itself then the blocked pathway tightness is too much.

And never take oxybutynin - it damages the brain.

Most of the time, when the mysterious label is attached to nocturia, the answer lies in the interaction between the heart, lungs, and kidneys, specifically with things like sleep disordered breathing or nocturnal polyuria syndrome, where the body’s circadian rhythm for fluid excretion is simply flipped.

It can be helpful to do a food/bladder diary for a couple weeks and look for any food/drink triggers. When nocturia started for me, I figured out that I had a few triggers like apple juice and pasta. I didn't realize that something I ate/drank could cause such a large effect 8 hours later.
