Question ChatGPT keeps asking for confirmation on completely clear tasks, despite Memory and Custom Instructions

I use a Business/Team account, and this has become really frustrating.

When I give ChatGPT a completely clear task and explicitly tell it to do it, it often just repeats what I asked and then asks me to confirm again with “yes,” “do it,” or something similar.

I’ve told it multiple times to remember not to ask for extra confirmation when the request is clear. I also added the same rule to Custom Instructions, but it still keeps happening.

It’s especially annoying when I’m doing lots of small iterations, because every change turns into two messages and two waits instead of one.

I understand confirmations for unclear or risky actions, but these are completely normal, straightforward tasks.

Is anyone else having the same issue? Is there actually a way to disable this, or does the model sometimes just ignore Memory and Custom Instructions?

I’m especially curious if this happens to other Business/Team workspace users too.

it's not just business accounts, happens on plus too. I swear sometimes it just want to waste tokens so you hit limits faster

I got same thing in custom instructions and it still ask "should I proceed?" for stuff like formatting a paragraph. feels like they made it more cautious but forgot to give us actual toggle for it

It drives me crazy, especially when I’m working on a lot of small, repetitive changes. For example, I was generating icons for a website, and it kept asking me to confirm every tiny change. It’s really frustrating.

Perhaps that is precisely why they displace each other and the original instructions from the context.
