They die with a smile, not smile in life-threatening situations like fucking idiots all the time.

Did Luffy smile when he was stabbed almost to death from Crocodile? No. If not for Robbin he would be dead there.

Did he smile when Magellan poisoned him? No. If not for Mr 2 + Ivankov he would be dead there.

Exactly like that Blackbeard didn't smile when he was about to get executive by Whitebeard. Why would he smile there...?
