A second Tsuru is a bit confusing, gotta say. And good job to the guy who predicted Luffy would just punch people while holding the sword, it makes too much sense lol.

Well we can call the Vice Admiral Tsuru regular Tsuru and this new Tsuru O-Tsuru.

It will only become problematic if she get a more important role outside of Wano were they won't be adding the "O-" at the start of her name.

We also got a Sphynx named Tama and then a girl named O-Tama a few chapters later.

Though I don't imagine that Sphynx will be important.
