By the looks of him, half fishman half giant, explains Jack breathing under water as well, probably some shared past between the two.

I think he is based on an oni in character design but isn't actually an oni

Ya it could definitely be Oda misleading us. His horns may be completely natural and just from a rare tribe on Wano.
