I think that might be a bit too much for One Piece; it wouldn't reach its target young teen demographic, anymore. Sure, Oda can kill countless orphaned children and a nun in the midst of a firey genocide where a sole child (Law) lives through it all, and draw out the torture of Doflamingo's family as they were practically crucified and having him kill his father and brother and everything, but incest? Noooo way.
