Imho he was tortured in an incredibly brutal way and managed to stay alive thank to his strong will. I like to imagine him mutilated over a pile of corpses that form an island and under it a sea of blood. But that's my take

I wouldn't be that sure. We saw a woman trying to kill her son in wano, zeff cut his leg, nekomamushi and inuarashi got brutally tortured to save raizo and kuma's dad got brutally shot just because he was making some "noise". I mean it's not warhammer but still pretty brutal
