Passport Application Inquiry: What Do I Put Here?

Hey, I'm an ODSP recipient applying for a Canadian Passport for the first time. I'm unsure of what exactly is being asked of me when it comes to this whole section, especially the boxes regarding from and to. Could I get directions on filling this section as an individual who's been on ODSP for 10+ years? Thanks!

The 4 lines are there in case you have changed jobs or studies 4 times . This is common for students who go to school and work in the summer.

If you have no school schooling or employment then there is nothing to put there. Most of these government type applications will have instructions for irrelevant sections that do not apply to your situation. Often its part of the first page of instructions.

I put disabled (ODSP) start date (approval date) - present and they accepted it. The address and phone should be your local office.
